A smooth PDO thread is a barb-free polydioxanone filament placed in a mesh or fanned pattern to stimulate collagen rather than reposition tissue. There are no anchors, so the thread does not pull; it provokes a controlled healing response instead.

That response thickens and firms thin, crepey skin over the weeks after placement.

Smooth threads work best on areas where the goal is texture, not lift: the cheeks, the neck, and the fine lines around the mouth. Practitioners often lay them in a grid to treat a whole zone at once.
